It’s always sunny in Philadelphia is back on screens, and the Gang has wasted no time proving that eighteen seasons in, nothing about Paddy’s Pub has gotten any less chaotic. The longest-running live-action sitcom in American television history continues to defy expectations. The premiere follows Frank and Sam’s wedding day, but true to the show’s tone, nothing goes as planned. It’s revealed that the couple had been intimate in a creek the night before the ceremony, exposing them both to flesh-eating bacteria. While Frank’s exposure proves less severe, Sam’s infection quickly turns fatal, and she dies from the bacteria eating through her spinal cord before the wedding even concludes. Dee brings Sam down the aisle as a puppet, and during the kiss, her head detaches entirely, sending the ceremony into gruesome, unmistakably “Sunny” chaos.
The second episode shifts focus to Dennis and Dee’s attempt to legally validate the marriage after Sam’s death, motivated entirely by the fortune she had promised to share with them. Their scheme predictably unravels after they fail to secure the proper marriage certificate and submit the wrong thumbprint for verification.
A Tribute Woven Into the Chaos
Alongside the shock and dark comedy, the premiere also included a heartfelt tribute to Lynne Marie Stewart, who played Charlie’s mother, Bonnie, and passed away in February 2025. Charlie’s storyline in the premiere, which involves carrying Bonnie’s ashes to the wedding, doubles as a quiet send-off for the character and the actress who played her, giving longtime fans an emotional beat within an otherwise outrageous episode.

Early reviews of Season 18 have been largely positive. Critics have praised the premiere for balancing absurd, over-the-top comedy with moments that feel true to the show’s long-running character dynamics. The two-episode debut earned strong marks for its willingness to lean into dark, body-horror-adjacent humor, including scenes involving Sam’s corpse, while still keeping the tone recognizably comedic rather than genuinely disturbing.
