Hep B vaccine side effects remain overwhelmingly mild, but a major federal policy shift in late 2025 has pushed the topic back into the national spotlight. The Centers for Disease Control and Prevention formally adopted a new hepatitis B vaccination policy for newborns after the Advisory Committee on Immunization Practices voted on December 5, 2025 to end the decades-old universal birth dose recommendation.
That change has left many parents and adults searching for accurate, up-to-date information about what the vaccine actually does to the body, how common reactions really are, and what has changed in official guidance.
What Changed With the Hepatitis B Vaccine Schedule
For more than three decades, the CDC recommended that nearly every newborn receive a hepatitis B shot within 24 hours of birth. That guidance began in 1991 and became a cornerstone of the U.S. childhood immunization schedule.
The December 2025 ACIP vote, approved 8 to 3, replaced universal birth dosing with what officials call individual-based decision-making, sometimes referred to as shared clinical decision-making. Under this model, parents of infants born to mothers who test negative for hepatitis B surface antigen now decide, in consultation with their pediatrician, whether and when their newborn receives the first dose.
Infants born to mothers who test positive for hepatitis B, or whose status is unknown at delivery, still receive the birth dose without any change. Acting CDC Director Jim O’Neill approved the policy shift, stating that the change restores informed consent to families whose infants face limited risk of exposure at birth.
For families who choose to delay vaccination, the CDC recommends starting the series no earlier than two months of age rather than at birth. The three-dose schedule otherwise continues largely unchanged, with a second dose typically following one to two months later and a final dose between six and eighteen months.
The American Academy of Pediatrics has pushed back strongly against the new policy. AAP President Susan Kressly said pediatricians are already reporting more parents declining the birth dose, warning the shift could lead to preventable infections in infants. The AAP continues to recommend universal birth dosing for all newborns regardless of the updated federal guidance.
Common, Mild Side Effects of the Hepatitis B Vaccine
The most frequently reported hep b vaccine side effects involve the injection site rather than the rest of the body. According to CDC and manufacturer prescribing information, these reactions typically appear within hours of the shot and resolve within one to two days.
Common local and systemic reactions include:
- Soreness, redness, or swelling where the shot was given
- Mild fever, generally under 100 degrees Fahrenheit
- Fatigue or general tiredness
- Headache
- Irritability, particularly in infants
- Loss of appetite

Serious allergic reactions, including anaphylaxis, are rare but possible with any vaccine. Symptoms can include difficulty breathing, swelling of the face or throat, rapid heartbeat, or hives, and they generally develop within minutes to hours of the injection. Anyone who has had a severe allergic reaction to a previous hepatitis B dose or to any vaccine ingredient should not receive additional doses without medical guidance.
Postmarketing surveillance has also recorded rare reports of neurological events following vaccination, including Guillain-Barre syndrome, optic neuritis, Bell’s palsy, and other demyelinating conditions. Federal health agencies and independent researchers have reviewed these reports extensively. Large-scale safety studies, including reviews conducted through the Vaccine Safety Datalink, have not established a causal link between the hepatitis B vaccine and chronic neurological or autoimmune disease. Reporting a reaction to a federal surveillance system does not by itself confirm that the vaccine caused it.
What Federal Safety Data Shows
Two federal systems track hepatitis B vaccine safety on an ongoing basis: the Vaccine Adverse Event Reporting System, known as VAERS, and the Vaccine Injury Compensation Program, known as VICP.
VAERS accepts reports of any health event following vaccination, whether or not the vaccine actually caused it. Anyone, including patients, parents, and providers, can submit a report, which means the database includes coincidental events alongside genuine vaccine reactions. Public health officials use VAERS primarily to detect unusual patterns that warrant further investigation, not as proof of causation.
The Vaccine Injury Compensation Program operates differently. It compensates individuals who file a claim and can demonstrate an injury plausibly linked to a covered vaccine, based on a table of recognized conditions and timeframes or on individual case evidence. As of recent program data, just over one thousand hepatitis B-related claims have been filed since the program’s creation, with a portion resulting in compensation. The program does not require proof that a vaccine definitively caused harm, only that the claim meets program criteria, which makes it a legal compensation mechanism rather than a scientific determination of cause.
Who Should Talk to a Doctor First
Certain individuals need a conversation with a health care provider before receiving the hepatitis B vaccine. This includes anyone with a history of severe allergic reaction to a previous dose, people with a yeast allergy, since some formulations are produced using yeast, and anyone currently experiencing a moderate or severe illness, who may be advised to wait until they recover.
Pregnant women are not excluded from vaccination and are, in fact, universally tested for hepatitis B during pregnancy under current CDC guidance, a practice officials cited as a key reason for narrowing the newborn birth dose recommendation. Health care workers, infants born to infected mothers, and people with chronic liver conditions remain firmly within groups for whom the vaccine’s benefits are considered to clearly outweigh its risks.
Why the Vaccine Still Matters
Hepatitis B remains a serious global and domestic health concern. The virus can cause chronic liver infection, cirrhosis, and liver cancer, and infants infected at birth face the highest risk of developing lifelong chronic infection. The World Health Organization has set a target of 90 percent vaccination coverage worldwide by 2030 as part of a broader hepatitis B elimination strategy.
More than a billion doses of the vaccine have been administered globally since its introduction, and public health researchers continue to credit it with dramatic declines in hepatitis B infection rates and liver cancer cases in vaccinated populations. Multiple studies conducted by the World Health Organization, the CDC, and independent medical societies have found no evidence connecting the vaccine to sudden infant death syndrome, autism, or multiple sclerosis.
Bottom Line for U.S. Families
The policy landscape around hepatitis B vaccination is shifting, but the underlying safety profile of the vaccine itself has not changed. Mild, short-lived reactions remain the norm, serious side effects remain rare, and medical organizations continue to debate the newborn scheduling change rather than the vaccine’s fundamental safety.
